Submission Instructions

Florida residents who have vaccines on file with Florida SHOTS may have their records pre-populated. Log in to the Student Self-Service Portal to see what immunizations are on file. If new information needs to be submitted, please select the appropriate immunization form and submit through the portal.

Please submit your immunization records through the Student Self-Service Portal at least three weeks prior to orientation and/or course registration. Please allow at least 1-2 weeks for your registration hold to clear after submitting your records.

Viewing Your Immunization History

Newly admitted students: Please allow at least 1 week from your UF acceptance/account creation for any Florida SHOTS records to pre-populate.

  1. Log into the Student Self-Service Portal
  2. Go to Immunization
  3. Click the green Print button to see what vaccines are already on file. If all of your vaccines are on file, you’re done!
  4. Check your One.UF account to see if your immunization hold has been cleared. Missing vaccines will appear on your To-Do Items.

Submitting New Records

If your vaccines are not pre-populated in the portal, please have the appropriate immunization form filled out and follow instructions below.

  1. Have a doctor’s office, clinic, or health department fill out the form.
    1. Record must be signed and dated by health care provider or office representative.
    2. Record must have an office stamp or business card of the signer.
    3. Corrections to the record must be initialed and dated by the health care provider or office representative.
    4. The document must be legible and in English. Illegible documents will not be processed.
  2. Please make a copy for your personal records.
  3. Log into the Student Self-Service Portal and proceed to the Immunizations page .
  4. Click on each dropdown menu to view options.
  5. If you have any of the menu options on your vaccine record, select the item to enter your dates. You will see your entered dates listed on the right side of the screen.
  6. Repeat step 5 for all menus.
  7. After you have entered all dates, click the blue Submit button
  8. You will be directed to a confirmation page to review the dates entered.
  9. Upload the immunization form and supporting lab reports. You must upload your records in order for the entered dates to be saved. Data entry and form upload must be completed in the same session.
  10. You will see a confirmation window of successful upload.
  11. Allow at least one week for verification to take place.
  12. Check your One.UF account to see if your immunization hold has been cleared. Missing vaccines will appear on your To-Do Items.
